High TPO Antibodies: Causes, Meaning, and What to Check Next
When thyroid peroxidase antibody levels come back high, the result can be difficult to understand without context.
TPO antibodies are immune proteins directed against thyroid peroxidase, an enzyme involved in thyroid hormone production. Elevated levels commonly occur when the immune system has developed an autoimmune response against the thyroid.
Hashimoto’s thyroiditis is the most common condition associated with elevated TPO antibodies. It can progressively reduce thyroid function.
Graves’ disease can also be associated with elevated TPO antibodies. Graves’ generally causes hyperthyroidism, so thyroid function testing is important for distinguishing the overall picture.
Another possible setting is postpartum thyroiditis. This condition develops after childbirth and can involve a temporary period of abnormal thyroid function.
However, not everyone with positive TPO antibodies has abnormal thyroid hormone levels. Some individuals have elevated antibodies while their thyroid is still functioning normally.
For this reason, TPO antibodies should generally be interpreted together with tests such as TSH and free T4. Symptoms and medical history also matter.
A high antibody level is not something to diagnose or treat based on the number alone. The key question is whether there is evidence of thyroid dysfunction or autoimmune thyroid disease requiring monitoring or treatment.
Understanding the causes of high TPO antibodies can make thyroid test results easier to discuss and understand.
